India at the Center of the Global Oral Cancer Crisis, Driven by Smokeless Tobacco and Areca Nut Use

India Leads Global Oral Cancer Cases

India is at the epicenter of a global oral cancer crisis, accounting for a staggering 83,400 of the 120,200 oral cancer cases reported worldwide in 2022, according to a recent study published in The Lancet Oncology. This alarming statistic underscores India’s significant contribution to the global burden of oral cancer, largely driven by the widespread use of smokeless tobacco and areca nut products.

Smokeless Tobacco: A Leading Cause of Oral Cancer in India

The study reveals that smokeless tobacco is responsible for more than 30% of all oral cancer cases worldwide, with India being the leading contributor in South Asia. Bangladesh reported 9,700 cases, Pakistan had 8,900 cases, and Sri Lanka recorded 1,300 cases. In contrast, Southeast Asia saw lower figures, with Myanmar, Indonesia, and Thailand together accounting for 3,375 cases. China in East Asia reported 3,200 cases, far fewer than India.

Dr. Pankaj Chaturvedi’s Insights on India’s Oral Cancer Crisis

Dr. Pankaj Chaturvedi, a renowned oncologist at the Tata Memorial Centre in Mumbai, pointed out that despite the gutkha ban, a widely used smokeless tobacco product, the tobacco industry has found loopholes to continue promoting and selling these harmful products. “Free accessibility and surrogate marketing have made India one of the world’s largest hotspots for oral cancer,” Dr. Chaturvedi stated, stressing that the tobacco industry contributes minimal revenue to the government compared to the enormous healthcare costs associated with treating the disease.

Bollywood’s Role in Promoting Areca Nut Consumption

The study also highlights the role of Bollywood endorsements in normalizing the use of areca nut, which is widely consumed in India despite its well-established links to oral cancer and submucous fibrosis. Dr. Chaturvedi emphasized the urgent need for stricter enforcement of regulations on these hazardous products, as their promotion continues to pose a major health threat.

The Widespread Use of Smokeless Tobacco in South-Central Asia

With over 300 million global smokeless tobacco users and 600 million consumers of areca nut, South-Central Asia remains a hotspot for both categories. In India, products like gutkha, khaini, and betel quid with tobacco are the primary causes of oral cancer, particularly among both men and women. The rampant use of these products continues to devastate public health across the region.

Call for Immediate Public Health Interventions

The findings of the study are a call to action for public health interventions to curb the use of smokeless tobacco and areca nut products. The harmful effects of these substances are severely impacting the health of millions and contributing to a growing public health crisis in India and South-Central Asia.
